JOB DESCRIPTION

Team: Transitional Reentry Services

Reports to: Transitional Reentry Services Program Director

Pay Rate: Established in offer letter

Service Area: New Castle County

Travel Required: Regular, New Castle County

Position Type: Full-time, exempt

Role and Responsibilities

The duties listed below are intended as examples of the types of work that may be performed by a Post Release Case Manager but are not all necessarily performed and do not include all specific functions and responsibilities that Post Release Case Manager may be expected to perform. 

  • Manage a caseload of individuals who have recently exited Delaware Department of Corrections Level 4 & 5 Facilities and receive Medication Assisted Treatment (MAT) for substance use.  
  • Provide in-depth, wrap-around case management services to individual clients referred to through the DE DOC’s Behavioral Health Provider. 
  • Perform reentry needs assessment with each client and create an appropriate recovery case plan. 
  • Conduct individual case management sessions with clients, both at intake and at regular intervals while cases remain active. 
  • Utilize a variety of interventions such as TCU Mapping, Cognitive Behavioral Therapy, and Motivational Interviewing to support client well-being and target criminogenic risk factors.   
  • Coordinate services related to recovery supports, housing, basic needs, transportation, education, job placement, and social supports/needs.  
  • Assist in client’s resource utilization by accompanying clients to off-site appointments or programs, as needed. 
  • Provide frequent client follow-up contact and support via phone, off-site visits, or office visits.
  • Communicate with the courts and probation and parole and advocate during court proceedings when needed.
  • Assess and report on client progress and participation at regular intervals.
  • Track and update client information using database program.
  • Generate monthly, quarterly reports and year-end reports to Dept. of Corrections and Management.
  • Assist with policy, communications, and development as they relate to the staff member’s assigned program.
  • Accept and perform other work as assigned.

Qualifications and Education Requirements

  • Significant knowledge and experience related to justice-involved people, the social forces and systems impacting them, and the resources available to them required. Bachelor’s Degree in human services, social work, or a related field also required. Master’s degree preferred. Experience in motivational interviewing, strengths-based case management, and/or Cognitive Behavioral practices also preferred.
  • 3-5 years’ experience performing case management is desirable.
  • Must demonstrate knowledge of substance use, mental health and co-occurring conditions, as well as working knowledge of resources available to individuals with SUD/Co-Occurring Disorders.
  • Ability to work independently and as part of a team.
  • Ability to multi-task and effectively manage work at different sites (in-office and off-site).
  • Must have a valid Delaware Driver’s License.
  • Background checks and clearances required as per program contract(s).
  • Basic computer skills, including but not limited to the use of Microsoft Office, as well as Zoom/Skype and other distance meeting resources.
  • Ability to multitask and work both independently and with supervision, on-site and remotely.
  • Embrace an understanding of justice involvement that acknowledges the impact of institutional racism and the systemic oppression of BIPOC, women, and LGBTQ+ people; must share the belief that people can change in positive ways.
  • Must embrace teamwork, have a strong work ethic, be a self-starter with a positive attitude, with a good sense of humor.
  • Strong team building, organizing, writing and other communication skills are essential.
